Patient Information

To begin the treatment process, we will need a new patient or family to fill out a “New Patient Information form”. These forms will be sent, via email or mail, and can be done before the first appointment to save additional time in the office. (All information that we obtain is personal and private. This information is protected by law and we take that very seriously. If you have concerns about information we have requested please discuss them with our office staff.) These forms cover medical conditions, current concerns, medications, allergies, medical and surgical history, insurance/payment information, and any information we might need to better care for you and your family. We will customize the treatment plan to the needs of the individual patient. We do our best to minimize your anxiety and give you a positive patient experience. We offer aromatherapy, heated neck wraps, hot tea or infused ice water, and a relaxing environment for your healthcare. We want your healthcare to be something that you look forward to participating in and would love to know if there is anything else we can do to make it positive for you. Children and healthy adults generally need to have one full examination a year. People with chronic healthcare problems may be seen monthly, quarterly, or as often as needed to keep your condition stable and your quality of life the best it can be.
